To request an Energy Assistance appointment, call ICS (448-4540) with the following information: your complete name, mailing address, phone number and social security number. Click here to determine if you are eligible. Appointments can be scheduled beginning October 2008 for the 2008-2009 season. You will receive a brief application in the mail with the time and date of your appointment.
Please bring the following information to your appointment:

Proof of Social Security numbers for your entire household. (Social Security cards, Medical assistance cards, Tribal ID cards and Pay check stubs only)

Proof of GROSS income from all sources for your ENTIRE household for the past three {3} months; the three months prior to your application date. {For example, if your appointment is on November 10, you must provide income verification for August, September and October.}

If you buy heating fuel, please provide evidence of your household's heating costs for the past 12 months.

A recent Wisconsin Public Service bill.

We also encourage you to bring a translator if needed.
We also have multiple Outreach Sites that you can schedule an appointment with if it is more convenient than coming down to our office.
